If the referral is accepted, you will be contacted for a brief phone call to discuss your goals for the evaluation and to collect additional information. You will be given information about the clinic policies, fees/billing, and what to expect during the evaluation. You will also have the opportunity to ask questions. An appointment will be scheduled (or you may be placed on a waitlist) if you would like to proceed.

• Cherry Hills Neuropsychology LLC is contracted with Medicare, Moda, PacificSource (commercial and Medicare), Providence, and Regence BCBS. If not listed, please contact us to determine if we are in-network with your insurance.
• If Cherry Hills Neuropsychology LLC is in-network with your insurance, we will obtain prior authorization and file for reimbursement. You will be responsible for paying any copays, co-insurance and/or deductibles at the time of your first appointment.

• You are responsible for paying for services at the time of the first appointment. You will be provided with a Good Faith Estimate prior to your appointment, which will summarize billing codes, diagnostic codes, and range of fees. You will be responsible for confirming coverage and submitting documents to your insurance company if you choose to seek reimbursement. Following completion of your evaluation, you will be provided with a “Superbill” detailing your billing and payment, which you can submit to your insurance for reimbursement if you qualify.
